项目摘要

(1)『外来受診時のコミュニケーション』に関するアンケート対象は(社)全国学校栄養士協議会の学校栄養士365名。医師の説明時に理解できない単語や言葉の経験:「あり」38%、「なし」57%、説明を十分理解できないがうなずいた経験:「あり」54%、「なし」42%、質問できなかった経験:「あり」64%、「なし」34%であった。尚、医療用語の調査では「CRP」を約8割がわからないと答えた。(2)小児科外来を受診した患児の診療前後における「あいさつ」の有無について円滑なコミュニケーションを図る上では「あいさつ」の役割は大きい。3歳以上の児49名を対象に診療前後の「あいさつ」等を検討した。入室時に「あいさつ」した児は17名、退室時31名であった。退室時に増えた理由としては、児・保護者ともに緊張がとれたことが考えられた。診療時の緊張感や警戒心、恐怖心を軽減することは、診察や検査をよりスムーズに行なう上で重要と考えられた。(3)当科外来診察室(平成18年11月開設)の新築設計における音響学的配慮とその実際外来診察室の遮音性、残響や共鳴に配慮して設計した。非診療時の処置室・診察室の暗騒音は約25dB・A、廊下は約49dB・Aで、診察室と待合室、処置室待機場所の遮音性能はD-20程度であった。また、診察室内の残響時間がやや長く、会話の明瞭度が低下する可能性が予測された。現在、実際の診療時における音響学的評価を行なっている。(4)病診・病病連携に関する検討患児の病状や家族のニーズに対して適切な診療を迅速に提供する上では、施設や職種を越えたコミュニケーションを図ることの重要性について論文としてまとめた。(5)母親支援に関する書籍「お母さんの悩みがなくなる『子育てナビ』」の発刊実際の家族からの声なども含め、これまでの診療経験をもとに患児やその家族とより円滑なコミュニケーションを図る上でのポイントなどについて触れた。
