精神治療領域におけるインフォームドコンセント(IC)に関わる熟練看護師の実践的知識について、6カテゴリー(関係性の構築、コミュニケーション、信頼性の構築、服薬の導入と協働、退院への環境調整、チームアプローチ)が前年度のグランデッドセオリーアプローチ(GTA)により抽出された。今年度調査は、当該力テゴリーに基づき質問項目を設定し、概念の妥当性、信頼性について質問紙調査により定量的に検証することが目的である。まず、内容妥当性に基づき生成された概念についてアイテムプール作成後、各力テゴリーに関連する質問紙を構成し、約100名の看護師に対する予備調査を行った。質問項目のワーディング等の再検証を行い、探索的因子分析により質問項目のconvergentおよびdiscriminant validityについて検討し、質問項目の精緻化を図った。その後、研究協力依頼に同意した精神科病床200床以上の16病院を選定し、精神科看護師713名を対象者として自記式質問紙の郵送回収による本調査を行い、535名から回答を得た(回収率約75%)。探索的因子分析により7概念(関係性の構築、コミュニケーション、信頼性の構築、服薬の導入と協働、入院体制の調整、治療計画(方針)の習熟と退院への連携、チームアプローチ)を抽出したが、各因子の因子負荷量や因子間の相関分析により、概ね妥当性は確保されているものと推察された。また、各概念領域のChronbachのα係数は、概ね0.8以上を示し、領域内の十分な内部一貫性が得られていることも示された。各領域の平均得点では、コミュニケーション、関係性の構築、服薬の導入と協働、チームアプローチの順に高く、当該領域に対する認知度・実践度が非常に高いことが明らかとなり、精神科看護領域における実践的知識として極めて重要であることが示唆された。

Psychiatric field related to practice knowledge of skilled nurses, 6 categories (relational construction, information construction, information construction, service drug introduction coordination, hospital environment adjustment, community planning), and previous year's change in practice (GTA). This year's survey is aimed at determining the validity, reliability, and quantitative aspects of the force based questionnaire. After the concept of content appropriateness was created, the questionnaire for each force was formed, and about 100 caregivers prepared for the survey. Question items such as re-examination, exploratory factor analysis, question item convergent and discriminant validity, question item refinement In response to a survey conducted in 16 hospitals with more than 200 psychiatric beds and 713 psychiatric nurses, 535 psychiatric nurses answered self-administered questionnaires (response rate: approximately 75%). The factor analysis of exploration includes seven concepts (relational construction, communication, information construction, introduction and coordination of services, adjustment of admission system, familiarity with treatment plan (policy), connection with hospital discharge, and communication), extraction, factor load, correlation analysis between factors, and general relationship assurance. The Chronbach alpha coefficient of each concept domain is greater than 0.8, and the internal consistency of the domain is greater than 0.8. The average score in each field is high, the relationship structure, the introduction and coordination of services, the order of services, the recognition and practice of services in this field are very high, and the knowledge and practice of psychiatric care are very important.
