项目摘要

歯科診療時の小児の不快情動を抑制する目的で,パイオニア社と共同試作した歯科用体感音響装置について,小児の充填処置時の内部行動変化の抑制効果を分析した.内部行動変化の記録は,日本光電制多用途監視記録装置を用いて,皮膚電気反射(GSR),耳朶容積脈波(PL),心拍数(HR),呼吸曲線(RC),眼球運動(EOG)の5指標について行った.この結果については,昭和62年度小児歯科学会総合において報告したが,内部行動変化(不快情動)の抑制率は,GSR44.3%,PL42.2%,HR28.3%,RC24.0%,EOG8.4%であり,いづれの指標においても,装置の効果が認められた. また,タービンやエンジンによる切削処置ではPLの変化が,乾燥や充填処置ではGSRの変化が,顕着に抑制されていた.61年度の質問調査や外部行動変化ならびに本年度の内部行動変化の分析結果をもとに,モリタ製作所とボディソニック社の協力を果て,歯科用診療ユットに体感音響装置を組み込んだものを製作し,上記学会で発表した.診療不適応児の心理学的背景に関しての検策は,小児の性格検査と親子関係診断検査を用いて行った.その結果,小児の性格では「顕示性」,「神経質」,「学校への適応」の各要因が,養育態度(親子関係)では,父親の「厳格型」,「干渉型」,母親の「不安型」,「溺愛型」の各要因が,不適応行動の生起に関連していることが明らかになった.また不適応行動のコントロールには,笑気吸入鎖静法や聴覚減病法(前述の体感音響装置を含む),トレーニング法やモデリング法などの行動標法的アプローチが有効であることも明らかになった.なお,本研究の成果は,今までの研究成果も加味して,「小児の歯科医療心理」として出版した.

The purpose of suppressing the unpleasant behavior of small children during dental diagnosis is to analyze the suppression effect of internal behavior variation during filling treatment of small children. The five indicators of the Japanese photoelectric multipurpose monitoring and recording device, GSR, PL, HR, RC and EOG, are used in the recording of internal movement changes. As a result, the children's personality is related to the factors of "personality,""mental quality,""school fitness," parenting attitude (parent-child relationship), father's "personality type,""dry type," mother's "anxiety type,""indulgent type," and inappropriate behavior.
