项目摘要

昨年度の研究成果に基づき,本年度は特に山間過疎地域の救急医療サービスに焦点を絞り,検討を行った.(1)昨年度開発した“地点別にみた傷病者の救命率"を内容とする救急医療サービス水準評価モデルを改良した.事例分析を行ったところ,地域住民が主観的に感じているサービス水準を比較的よく表現することが確認された.また,山間過疎地域では地域内においても集落間でサービス供給水準に大きな格差がみられること,いくつかの支配的要因が存在することなどが明らかとなった.(2)治療サイドからも傷病者の側へ出向くことが救命率を高める上で効果的であることから“ステージ別評価"の考え方を導入し,搬送・治療プロセスの時間的前後関係に着目した整備方策検討手順を開発し,搬送と治療の有機的連携により一定のサービス水準の向上が図れること,地理的特性等の故に効果的なシステム整備方策が地点によって必ずしも同一でないこと,などの結果が得られた.(3)救急医療を支える公立医療施設の経営モデルと家計の行動モデルを構築し,経営成立性を確保するために必要となる財政補助金額ならびに家計の支払い意志額の算定を通じて,病院の存続が社会的に認められる条件を検討した.関係自治体による補助金(住民の負担金)と施設規模から見た公立病院の維持可能性を検討した事例分析では、人口密度が低くてもサービス圏域が広くかつ一定のサービス水準を提供しうる場合には,公立医療施設の維持が可能であるが,そうでない場合には極めて困難であることなどの示唆が得られた.以上の検討を通じて,山間過疎地域の緊急安全サービスに関するさまざまな有用な知見が得られたが,これらをもとに,今後は過疎地域の計画論の確立に向けてさらに研究を発展させていきたいと考えている.
