The purpose of this study is to clarify the relationship between depressive states and eating disorder among Japanese adolescents, using self-administered questionnaires. Attempts on revision of the depression scale to improve the validity of screening with the findings obtained in pilot studies were also tried and newly revised questionnaires were used in the main studies. Study subjects were 485 junior high school students and 159 university students in pilot studies. The DSD based on the DSM-IV criteria and the EAT-26 were used for assessing depressive state and eating disorder, respectively. Factor analysis on the pilot data revealed strong relation between 'appetite'/weight change' items of the DSD and several bulimic items of the EAT-26. Thus, the DSD was revised with bulimic items from the EAT26 for the main studies. After the revision of the questionnaires, 480 junior high school students, 164 senior high school students and 141 university students were participated in the main studies. The 30 outpatients in a clinic for chills and adolescents psychiatry also participated in the questionnaire survey, with clinical diagnostic interview of the modified SCID. The agreement (kappa coefficients) on the MDE (Major Depressive Episode) diagnosis by the DSD and the modified SCID became a little bit higher in the clinical samples after the revision, although the finding may need to be confirmed in a large sized general population. Both among the general and clinical samples in the main studies, a strong association was found between depressive state and eating disorder symptoms, especially for girls. This tendency was prominent in the clinical samples, but in the general samples it seemed to be emphasized as the grade increased.
