本研究では、固形腫瘍とくに悪性リンパ腫と肺癌症例の末梢血幹細胞移植(PBSCT)に関して、基礎的検討と臨床的検討を平行して行った。基礎的検討に関しては、末梢血幹細胞採取凍結保存検体を用い、トロンボポエチン(TPO)によって刺激培養した後の検体に含まれる各種細胞の電顕ペルオキシダーゼ(PO)活性を検討した。TPO添加検体では、MPO陽性の細胞も観察されたが、G-CSF添加検体に比べると頻度的な少数であった。一方、PPO陽性の細胞は、TPOおよびIL-3添加検体のみで検出された。細胞表面マーカーの解析では、サイトカイン非添加の検体に比べTPO添加検体では、CD13・HLA-DRの軽度の増加が認められ、またCD41aおよびCD42bについては著明な増加が観察された。以上の結果より、TPOは、末梢血幹細胞に対して、血小板膜糖蛋白や電顕PPO活性の発現を単独で誘導しうると考えられた。臨床的検討に関しては、PBSCT後の免疫系の変動につき、リンパ球サブセットと血中サイトカイン量を測定し、悪性リンパ腫と肺癌との比較検討を行った。移植後のリンパ球サブセットの変動としては、activated T cell(CD^<3+>HLA-DR^+およびCD8^+HLA-DR^+)とsupressor/cytotoxic T cell(CD8^+CD11b^-)の上昇が肺癌において著明であった。さらに、NK cell(CD16^+CD56^+,CD3^+CD16^+)は、肺癌と悪性リンパ腫とで同程度の増加が認められた。サイトカインと可溶性因子については、IL-6およびsIL-2Rの上昇が肺癌において著明であった。以上より、成人固形癌特に肺癌において、PBSCTをLAK療法として位置づける可能性が示唆された。

This study was conducted in parallel with the study of peripheral blood stem cell transplantation (PBSCT) in patients with lung cancer, including primary and secondary lung cancer. For basic research, peripheral blood stem cells were used in cryopreserved models, and the electrical conductivity (PO) activity of various cells contained in the models after stimulated culture with TPO was investigated. The number of TPO and MPO-positive cells detected was lower than that of G-CSF. One side, PPO positive cells, TPO and IL-3 were detected. Cell surface analysis is different from TPO addition, CD13 HLA-DR enhancement is different from CD41a and CD42b enhancement. The results showed that TPO, peripheral blood stem cells, platelet membrane glycoprotein and PPO activity were induced independently. Clinical evaluation related to the immune system after PBSCT, including the determination of serum levels, and comparative evaluation of lung cancer. After transplantation, there was a marked increase in the number of activated T cells (CD3 +>HLA-DR^+ CD8^+HLA-DR^+) and suppressor/cytotoxic T cells (CD8^+CD11b^-) in lung cancer. In addition, NK cells (CD16^+CD56^+, CD3 ^+CD16^+) increased to the same extent as lung cancer. Soluble factors such as IL-6 and sIL-2R increase in lung cancer. The above results indicate the possibility of PBSCT treatment for solid cancer, especially lung cancer.
