1.地域医療施設や社会教育施設などの地域公共施設は, 住民のニードを充足し, 効率的かつ平等にサービスを提供することが要求されるが, 実際には相当の地域間・地域内格差が存在している. 一般に施設の利用効率を高めようとすれば, 集中化により格差は拡大し, 格差を縮小しようとすれば利用効率は低下する. そこで, 本研究は利用者の施設利用行動にどのように反映しているのか施設水準の格差の実状を調査分析し, 施設配置計画における平等性と効率性のトレードオフに関する計画指標を明らかにすることを目的としている.2.研究方法としてはまずモデルによる理論的解析を行い, 事例として公共図書館と地域医療施設を選び, このモデルの妥当性を検討するという手法をとった. 利用者の属性と施設水準の2つの要因が利用行動に与える影響をそれぞれ別個に計量する手法として, ランダム効用理論にもとずく多項ロジットモデルが有用であることが明らかになった.3.第一の事例としては千葉県柏市において地域図書館システムに関する住民の利用状況調査を実施し, 施設水準の格差が施設選択行動にどのように影響しているかを多項ロジットモデルを用いて分析した. 住民属性としては, 自家用車の保有, 自宅における蔵書数, 勤務先などが有意であり, 施設水準指標としては図書館の蔵書数, 図書館までの距離が有意であるとの結果が得られた.4.第二の事例としては広島県の患者調査のデータを用い, 主として地域間格差についての分析を行った. 市町村間の年令構成による影響を排除した訂正患者率には施設水準の影響力はあまり明らかではない. これが新患発生率と平均入院期間とどのような関係にあるのかは, 一日断面調査である患者調査からは明らかにはできなかった.
