【方法】1.同種移植による気管再建:雑種成犬22頭(8-13kg)を対象とした。2頭同時手術とし開腹、摘脾後に有茎大網片を作製し右胸腔内に誘導、右開胸後胸部気管9軟骨輪を切除した(切除長3.5-5.5cm)。摘出気管の両端2軟骨輪は残し、中央5軟骨輪を除去し交換移植した。吻合後、管腔保持のため軟骨除去部にhydroxyapatite製馬蹄型人工軟骨輪を、2個留置した後、有茎大網片で被覆した。移植翌日よりFK-506 0.1mg/kgを筋注投与した群をA群。非投与群をB群。対照として軟骨輪を除去せずに交換移植し有茎大網片で被覆、FK-506 0.1mg/kgを投与した群をC群とした。各群とも経時的に内視鏡による観察とレーザー組織血流計による粘膜血流測定を行った。また、各群の数例に対し右胃大網動脈より墨汁を注入し移植片粘膜面を観察した。2.同種気管の背部皮下埋め込みによる線毛輸送機能の検討:数頭の雑種成犬で試みたが、粘膜面の乾燥、痂皮化のため評価不能であった。【結果】A群;9例中6例は狭窄することなく移植片は生着したが、1例は5週目に狭窄にて死亡した。2例は合併症にて早期に死亡した。B群;9例中6例は移植片狭窄を認め、1例を除く5例は21から50日目に死亡した。3例は合併症にて早期に死亡。C群;全例早期に移植片粘膜壊死をきたし、19から37日目に狭窄死した。レーザー血流計による移植片中央部血流/宿主側気管血流の割合(G/N値)は、各測定時期においてA群がC群より有意に良好であった。免疫抑制剤投与下でも、本法により移植気管の延長が可能と結論づけられた。今後、移植片の超冷凍保存による脱抗原化を組み合わせ、研究を展開する予定である。

[Methods] 1. Homotransplantation: 22 adult dogs (8-13kg) were used for reconstruction of allografts. 2 heads were operated at the same time. After splenectomy, there was a large net of stem. After right thoracotomy, the chest tube was induced. After right thoracotomy, the cartilage wheel was removed (the resection length was 3.5-5.5cm). The 2 cartilage rings at the end of the tube were removed and the 5 cartilage rings at the center were removed and transplanted. After anastomosis, the lumen is kept and the cartilage is removed. The horseshoe artificial cartilage wheel is made of hydroxyapatite. After 2 detentions, the stem is covered with large mesh. FK-506 0.1mg/kg was injected into the muscle the next day. Non-projection and group B. For example, if the cartilage wheel is removed, the graft will be covered with FK-506 0.1 mg/kg. The blood flow of each group was measured by endoscopy. The mucosal surface of the graft was observed after injection of ink into the right gastric aorta in several cases. 2. A study on the function of hair transmission under the skin of the same kind of tube: several kinds of adult dogs try to dry the mucous membrane surface, and the evaluation of crusting can not be done. [Results] Group A: 6 of 9 cases had stenosis and 1 case died after 5 weeks. 2 cases died of complications. Group B: 6 cases of graft stenosis, 1 case of graft stenosis, 5 cases of graft stenosis, 21 cases of graft stenosis and 50 cases of graft stenosis. 3 cases died early due to complications. Group C: All patients died of mucosal stenosis on the 19th and 37th day after transplantation. The blood flow in the central part of the graft/the blood flow in the host side (G/N value) were measured in groups A and C. Conclusion: Immunosuppressive agents may be used to prolong the graft. In the future, the preparation and development of antigenization for cryopreservation of allografts will be determined.
