(1)原発閉塞隅角緑内障、原発開放隅角緑内障および新生血管緑内障の患者に対してトラベクレクトミ-を施行。その際に得られた約1×3mm大の隅角線維柱帯組織を半割し、光学顕微鏡用および電子顕微鏡用切片を作成した。(2)光学顕微鏡用標本のHE染色にての所見であるが、原発開放隅角緑内障では線維柱帯ぶどう膜網、強膜網には正常眼と比較して大きな差異は認められなかったが、傍シュレム管組織にはやや密な形態で、シュレム管直下の組織はやや板状な部分も認められた。原発閉塞隅角緑内障では線維柱帯ぶどう膜網、強膜網がその板状構造がやや変形し、さらに線維柱間の間隙は狭くなっていた。しかし、シュレム管には特に変形などは見られず、ほぼ正常であった。新生血管緑内障は原発閉塞隅角緑内障と類似した構造であったが、線維柱は一部では完全に閉塞し、新生血管および血球が認められた。(3)酵素抗体法では平滑筋由来α-アクチン収縮蛋白が線維柱帯ぶどう膜網に存在し、原発閉塞隅角緑内障では原発開放隅角緑内障に比してやや染色性が高かった。また、新生血管緑内障ではさらに染色性が高く、とくに癒着や新生血管の見られる付近には特にその傾向が高かった。(4)以上の結果を人眼の正常発達期の隅角線維柱帯組織と比較すると、傍シュレム管組織がやや厚みがあるのは隅角の未熟性を反映しており、原発開放隅角緑内障にはその傾向があるとも考えられる。平滑筋由来α-アクチン収縮蛋白は、毛様体筋の発達に従い隅角線維柱帯に到達するが、この点では原発開放隅角緑内障はほぼ正常であった。原発閉塞隅角緑内障や新生血管緑内障でやや強く染色されたのは、炎症性の変化の結果の可能性もある。
