本研究の本年度の目的は、初年度から継続し海外で国際保健医療活動をした看護者に面接調査を行い昨年度報告した政府組織から派遣された看護者と異文化適応過程の特徴と非政府組織から派遣された場合の異文化適応過程の相違を明らかにすること。および、今後の本研究を異文化適応過程の数量的分析を視野に入れ"St.Clair"らの作成した文化エフェカシー尺度の日本語版の作成および信頼性の検討を行うことである。日本語版作成に関しては、翻訳を続けている状況であり報告するに至っていない。非政府組織から派遣された看護者への面接調査は10名を対象に半構成的枠組を用いて実施した。対象のそれぞれの背景要因を非政府組織と比較すると、参加スタイルは国際組織へ出張という形で参加し、動機は個人の興味関心に加え組織的な活動の一貫であった。また、活動の特徴は、プライマリーヘルスケア活動(3名)、国内外の紛争による後方病院での医療活動および病棟管理(7名)であった。活動員の構成は、国際多国籍チーム員であったことも背景に相違があった。また、派遣期間は組織の方針として6ヶ月を原則としていた。看護者の異文化適応のプロセスにおける非政府組織看護者の適応に影響した出来事は、住居地周囲の治安が不安定であること、繰り返し紛争による外傷で医療を受けるという循環を俯瞰したことによる無気力さ無意味さ役割期待への疑念や援助者としてのアイデンティティを揺らぎであった。また、語学能力不足や多国籍組織ではチーム員同志でのサポートは難しく国籍や民族の違いによる差別をうけたという気持ちを抱いている看護者が少なくなかった。これらの出来事を受け入れ、あるいは解決し適応していく上でのサポートには「ケアの対象者」との関係性で確認した「自己存在の必要性の確認」であり、同時に「自分の肯定的成長の自覚」であった。異文化適過程の概観は、出発前の準備期間から帰国後の間で7段階の過程を経ていた。今後の方向性として、面接調査を中心に進めてきたことで異文化適応のコアとなる心理的要因の存在(使命感に類似する役割観念、援助者としてのアイデンティティ、参加の動機)することが推測できる。これらを明らかにすることを同時に行うと共に、基礎教育における国際保健医療活動における看護の教育を検討していきたい。
