老年在宅酸素療法適応者に対する在宅ケアシステムの構築と看護職の役割

本年度は最終年度として訪問調査を実施し、結果の分析、考察を行った。在宅酸素療法(以下HOT)適応者とその家族3事例を対象に、研究者が作成した調査用紙を用いて面接調査を行った。HOT適応者は、介護保険を申請せず医療保険のみ利用する場合があり、予定より調査対象数が減少した。調査した結果、HOT適応者の介護保険制度利用において、次のような問題が明らかになった。(1)労作時呼吸困難による日常生活困難と病状管理の必要性が調査に反映されず、要介護度が軽く判定される。(2)必要なサービスの量と質が不足している。(3)介護認定が病状変化に間に合わない。(4)医療機関や患者会から提供される情報は医療面に偏り、介護サービス情報が不足している。(5)通院困難のため往診医が必要である、(6)HOTの医療管理可能な医療機関が少なく、HOTを受けながらの他科受診が困難である。これらの結果から、HOT適応者の在宅ケアシステムには次の7点が必要と思われる。(1)介護サービス提供者・ケアマネジャーヘHOT適応者の理解を促すこと、(2)効率的な介護情報提供のしくみ、(3)医療と介護サービス提供者間の連携、(4)病診連携(入院先と往診医)、(5)HOTの医療管理可能な医療機関が増えること、(6)対象者のニードを捉え、行政施策へつなげること、(7)不足する公的サービスを補うインフォーマルサービスの活用。以上をふまえ、看護職には、HOTとその適応者への理解を促す教育、サービス活用へ向けた情報提供、個々のHOT適応者の医療・介護サービス調整と統合、さらに、医療、福祉、地域住民を含む有機的地域ケアシステムを構築する役割が求められると考察された。
