チームアプローチによる唇顎口蓋裂患者に対する治療の重要性が認識されて久しく、実際に多くの施設で、関連する各専門科の緊密な連絡のもとに総合一貫治療が行われている。したがって、唇顎口蓋裂患者の機能的、形態的な諸問題はかなり改善されるようになってきた。しかし、乳幼児から成人に達するまでの長期間において行われる治療がすべての施設で同じと言うわけでなく、治療方法や治療時期、治療効果の認識において施設間にかなりの違いがあるのも事実である。また、以前に行われた治療が患者の成長発育に全く無意味なものと成ったり、今後行うべき治療の障害になることもある。本研究の目的は、本邦において行われている唇顎口蓋裂のチームアプローチによる治療体系を見直すことであることから、関連施設へのアンケートによる調査を行った。調査内容は、生後の哺乳指導から、初回形成手術、言語、矯正、修正手術までの120項目にわたる設問項目に関して調査した。特に、鼻咽腔閉鎖機能不全に対するスピーチエイドと咽頭弁の有効性の比較、顎裂部への2次的骨移植の移植骨の種類と移植時期、残遺孔の閉鎖方法と閉鎖時期、外科的矯正治療の移動量と上唇組織量との関係に関しては、詳細に調査した。結果については現在解析中であり、平成11年度の日本口蓋裂学会に報告予定である。また、佐賀医大歯科口腔外科における顎裂部への二次的骨移植後の歯の萌出に関する研究結果と、レーザー三次元形状計測装置を使った顎矯正手術による顔面形態の分析結果、長崎大学歯学部第一口腔外科における口蓋裂児の言語と顎発育に関する長期観察例を、それぞれ専門学会雑誌に発表した。

The importance of treatment for cleft lip and palate patients has been recognized for a long time, and there are many facilities and relationships between various departments. Close contact and integrated treatment are carried out. The functional and morphological problems of cleft lip and palate patients are improved. For example, in the case of infants and young adults, there is a long-term relationship between treatment and treatment, treatment method and treatment period, and treatment effect recognition. The patient's growth and development is completely meaningless, and the future treatment is harmful. The purpose of this study is to investigate the treatment system of cleft lip and palate in China and to investigate the relationship between the treatment system and the treatment system. The survey was conducted on 120 items related to nursing guidance after birth, primary surgery, speech, correction, and revision surgery. In particular, the relationship between the type of bone graft and the time of transplantation, the method of closing the residual hole and the time of closing, the amount of surgical correction and the amount of upper lip tissue was investigated in detail. The results of the analysis are set in the report of the Japanese Academy of Sciences in Heisei 11.
