项目摘要

1970年代後半以降、途上国における国民の健康状態の向上のためには、プライマリーケアがもっとも有効なアプローチであると考えられており、他方で急速に発展している民間医療セクターをプライマリーケアに取り込んで行くことが求められている。このため、本研究では、一国の保健政策として、民間医療セクターをプライマリーケアに活用するための諸方策について検討することを目的としていた。本研究の第一段階における実績としては、まず、明治時代の漢方医団体と当時の政府の関係、さらには第二次世界大戦後の日本医師会と厚生省の関係を法律・学術上の資料より調査した結果、昭和前半の健康保険導入はプライマリーケア活動における公と民の連携と統合に決定的な影響を与えたことが明らかになったことである。健康保険の導入の結果、プライマリーケアの第一の提供者として、医師の地位は他の職業(例えば薬剤師)に比べて大幅に強化されたことが判明した。さらに、日本医師会の性格はたんなる同業者団体から「査定」というプロセスを通して、医師ひとりひとりの診療行為をチェックする管理組織に大きく変わった。この結果、診療の質的改善へのさまざまな動きが強まり(例えば医師のための「診療方針」、患者のための「受診心得」の作成など)、プライマリーケアの水準が引き上げられた。プライマリーケアの改革の動きは、さらに、後の国民健康保険の導入によって、一気にスピードを増したことが明確になった。さらに、上記に述べたような日本の諸政策の検討から考えられる政策選択肢の、途上国のプライマリーケア政策への具体的な活用の可能性について検証を進めるため、予備調査として、東南アジア地域の途上国についての資料を収集するとともに、現地の研究者との連絡がなされた。

In the second half of the 1970s, the health status of the people was improved, and the development of folk medicine was accelerated. This study aims to improve the health policy of a country and improve the utilization of various policies in private medicine.
