β遮断薬には日本人に特徴的な変異が存在するCYP2D6の代謝を受けるものが多い。さらにβアドレナリン受容体には循環器系と関係のある機能の変化を伴う遺伝子多型がβ1,β2の型に存在する。そこで大阪府病院協会の協力によりCYP2D6による代謝が関与しているβ遮断薬が投与されている高血圧症の患者を対象として薬物反応性の個人差を調査することとした。このためβ1,β2受容体の遺伝子多型の判定法を確立し、これと同時に日本人に多いCYP2D6^*10AとCの酵母による発現系を確立し、これを用いてCYP2D6の基質となるβ遮断薬に対しての代謝活性を測定することとした。糖尿病患者についてβ受容体遺伝子多型を判定した。β1受容体のArg389Glyでは141名の検討ではワイルド群82名(59.9%)、ヘテロ群53名(36%)、ホモ変異群6名(4.1%)であった。β2受容体については174名の検討でArg16Glyではワイルド群49名(28.2%)、ヘテロ群89名(51.1%)、ホモ変異群36名(20.7%)であり、Gln27Gluにつてはワイルド群146名(83.9%)、ヘテロ群28名(16.1%)、ホモ変異群名(0%)であった。遺伝子型と臨床検査上の関連はArg16Glyでは女性においてBMIが変異を持つ群(25.17±3.7)のほうが有意に持たない群(22.17±3.9)より大きく、Gln27Gluではへテロ群(230.7±278.7mg/dl)がワイルド群(141.5±86.9mg/dl)に対して有意に中性脂肪が高値であった。発現系による検討ではCYP2D6の基質となりβ遮断薬であるBufuralolと抗うつ薬であるVenlafaxineを用いて代謝活性を測定した。Vmax/KmはCYP2D6^*1>CYP2D6^*10A>CYP2D6^*10Cであった。さらに^*10酵素の活性の低下は酵素の不安定性ならびに低発現であることも明らかとなった。今後β遮断薬の投与されている患者を対象として検討をおこなう。

β-blockers differ from Japanese characteristics in the presence of CYP2D6 metabolism. A method for determining the genetic polymorphism of CYP2D6^*10A and CYP2D6^* 10C receptors in Japanese was established and the metabolic activity of CYP2D6^*10A and CYP2D6 ^*10A was measured. Diabetes mellitus patients are diagnosed with beta receptor polymorphism. Arg389Gly of β1 receptor was found in 141 cases, 82 cases (59.9%) in group A, 53 cases (36%) in group B and 6 cases (4.1%) in group C. 174 β2 receptors were detected in Arg16Gly, 49 (28.2%), 89 (51.1%), 36 (20.7%), Gln27Glu, 146 (83.9%), 28 (16.1%) and 0%(0%).
