Basic and clinical research of endovascular therapy for intracranial atherosclerosis lesions

1)Patency of the perforation artery after stent placement for the intracranial artery : In atherosclerosis induced rabbits, the orifice of the lumbar artery showed narrowing due to the proliferation of the intima around stent strut. However, most of cases, the lumbar artery was patent. These changes were different from those seen in normal rabbits.2)Results of clinical cases: 31 cases with intracranial atherosclerotic stenosis were treated with stenting. In three cases with total occlusion, one case showed re-occlusion in spite of the successful stenting and two cases showed restenosis. The rate of abnormality demonstrated in diffusion weighted MRI was small in number compared with carotid stenting. In 26 cases, long-term patency and stroke prevention was confirmed. Form these points, intracranial stenting was safe and effective.3)PVA coated covered stent : VEGF and bFGF were dissolved in the PVA polymer and released after stent placement. However, these materials were released within 24 hours. Slower release may be preferable for clinical use.
