项目摘要

口唇口蓋裂の治療では、良好な言語機能と顎発育という相反する課題について、いかに効果的に獲得可能であるかを焦点として、これまでにも数多くの手術方法および管理体制が報告されてきた。その中で、二段階口蓋形成手術法は、あえて口蓋部の手術を2回に分け、硬口蓋部の手術時期を遅くかつ侵襲を小さくすることにより顎発育抑制が少なく、形態的な面では良好な結果が得られるとされている。しかし、言語機能に関しては手術方法、硬口蓋閉鎖の時期など未だ一定しておらず、必ずしも十分な結果が得られていないとされている。二段階口蓋形成手術法の言語獲得過程を知る上で、硬口蓋閉鎖前の4歳から5歳時の言語を評価することは重要と思われる。そこで今回、新潟大学医歯学総合病院口腔外科・顎顔面外科診療室において、Furlow法によるHotz床併用二段階口蓋形成手術法を施行した症例で、軟口蓋形成術以前の早期から定期的に言語管理が可能であった4歳児37例(F4群)、5歳児33例(F5群)を対象(F群)に、鼻咽腔閉鎖機能、正常構音の獲得過程および異常構音について調査した。さらに、Furlow法による言語機能獲得過程の特徴を調査する目的で、Widmaier変法により軟口蓋形成術を行った症例(W群)と比較し、以下の結果を得た。1.鼻咽腔閉鎖機能は、4歳時、5歳時ともにF群においてW群より高率に、かつより早期に獲得されていた。2.正常構音の獲得例については、F群では異常構音の発現のない正常な構音発達を示す症例が多く、異常構音が出現した場合でも自然消失する症例が多かった。また、4歳時、5歳時ともF群の異常構音の数はW群より少なく、会話明瞭度はより良好と思われた。以上より、4歳時、5歳時の鼻咽腔閉鎖機能と構音は、F群はW群より良好であると考えられた。

Early stage of soft flap formation was performed. Regular speech management was possible. 37 cases were 4 years old (F4 group), 33 cases were 5 years old (F5 group). The acquisition process of normal articulation and abnormal articulation are investigated. To investigate the characteristics of speech function acquisition process in Furlow method, Widmaier method was used to investigate the symptoms of soft flap formation (W group) and the following results were obtained. 1. Nasopharyngeal cavity locking function: F group at 4 years old, W group at 5 years old, high rate at 4 years old, early stage at 5 years old 2. In the case of acquisition of normal articulation, F group, occurrence of abnormal articulation, occurrence of normal articulation, occurrence of abnormal articulation. The number of abnormal sounds in F group at 4 years old and 5 years old is less than W group, and the conversation is clear. The function of closing the nasopharyngeal cavity at the age of 4 and 5 is to form a sound, and the F group is to form a sound.
