保健師が統合失調症者の家族へ行う援助のうち、家族を生活者と捉え集団の場と家族とを結びつける援助の実際の特徴を明らかにするため、北海道立保健所の熟練保健師10名に半構成的面接を行った。得られたデータは逐語録化し、どのような状況の家族を援助対象と判断し、どのような工夫のもとどんな方法で介入するかについて意味ある文・句を抽出し、質的分析を行った。保健師は【療養者の病状が回復期以降の時期】、【家族員の興味・関心が外部へ向かう時期】、【家族員が参加への戸惑いを感じている時期】や、【家族の表現的機能に偏りがある】かについて査定し、そのような状況にある家族を援助対象として捉え支援を開始していた。そして、【地域の社会資源に自ら参加し療養者から対象を把握する】、【訪問前に知識や家族のイメージを十分膨らませる】ことを行った後、援助の必要な家族に対して、【家族員の療養者に対する感情を共有し本音を語ることのできる関係を作る】、【家族員の病気の受け止めや知識を確認する】、【療養者の安定した生活継続理由を家族員が考える機会を提供する】、【家族員の集団場面参加の決断への迷いに対して参加め後押しをする】、【参加勧奨が保健師の独り善がりにならないよう援助の受け止めを見極める】、【参加の勧め方を家族の気持ちに合わせる】、【学習会を教育の場以外に活用する】ことを行っていた。また参加者には【家族員の参加したことで生じた感情を後日確認する】こともしていた。今回の結果から、統合失調症者の家族を生活者として捉え援助する際、地域の資源を十分把握し、家族を一つの単位として捉えることが必要であると考えられる。また、家族員の複雑な心理に寄り沿った関わりの重要性が示唆された。今回援助者側からの研究としたため、今後は、家族員が集団の場への参加に至るまでに経験している戸惑いの観点から援助のあり方を検討していきたい。
