血液透析患者は、透析への移行過程で、知識を身につけそれを徐々に実践して新たな生活サイクルを習慣化していく。しかし、この段階で適切な生活習慣が身につかず、不適切な生活が習慣化してしまった場合、後にそれを修正するには大きな労力と時間を要する。従って、透析導入初期の段階で適切な行動を習慣化することは重要である。また、この時期は透析導入になってしまったことでの精神的落ち込みが大きいことや、不均衡症侯群など透析による身体的影響も大きいことから、患者が新しい生活スタイルを獲得するために効果的な看護援助を行うことが大変重要となってくる。本研究の目的は、3年間で、血液透析導入期にある患者の透析受容過程を明らかにすること、そこから血液透析患者が早期に透析に適応できるような看護介入プログラムを作成しその効果を検証すること、また効果判定のためのセルフケァ測定尺度を作成することである。今年度は、患者が身体的、精神的にどのように透析に適応していくのか、その過程を明らかにし、各段階にあった看護援助は何かを検討することを目的として面接調査を実施した。対象者は、血液透析をはじめてから5年以内の外来血液透析患者である。現在13名の対象者に面接を実施したところであり、現時点での対象者の年齢は平均56.9(範囲27〜74)歳、血液透析歴は平均21.3(範囲8〜47)ヶ月、職業は有職者6名、無職者3名、主婦4名である。現在録音した面接内容を逐語録にし、グラウンデッドセオリー法を用いて分析している途中である。最終的には血液透析患者の透析受容過程の概念モデルを作成する予定である。来年度は、本研究の結果を基に、血液透析導入期の患者に対する看護介入プログラムを作成しその効果を検証していく方向である。
