〔目的〕1998年ごろよりドメスティック・バイオレンス(DV)に関する調査は増加してきているが、病院を受診するDV被害者の実態および受け入れる病院の状況を調査したものはほとんどなく、ケアニーズアセスメントもなかった。そこで病院を受診するDV被害者のうち、特に夜間救急外来を受診するDV被害者の現状について参加観察し、ケアニーズアセスメントを作成した。〔方法〕3施設において外傷で夜間救急外来を受診した女性患者の中でDVが疑われる患者を対象に、患者の現状および医療職者側の現状について参加観察法を用いた質的研究を実施した。データは帰納的、記述的に内容を分析した。〔結果〕調査回数は102回であり、11人のDV被害者に対して参加観察を実施した。DV被害者の現状は、「外傷による身体的苦痛がある」「暴力を受けたショックによる動揺がある」「暴力の原因は自分にあると思い、罪責感を持っている」「自分で解決せざるをえない」他12カテゴリーにより構成された。それらは「身体面に及ぼす影響」「精神面に及ぼす影響」「加害者のコントロールによる影響」「帰宅後の生活に及ぼす影響」の4側面に大別でき、外傷の治療だけでなく、帰宅後の安全な生活に対するケアや、加害者や子どもへの対応も必要であることがわかった。医療職者の現状には、「泣いている患者の対応」「加害者が同伴している時の対応」など患者個々に対するケアと「ゆっくり対応できない」「時間がない」などシステム上の問題があり、対応に苦慮していた。〔結論〕救急外来を受診するDV被害者のケアニーズアセスメント作成には、患者の身体面や精神面に関するもの、加害者との関係に関するもの、患者の帰宅後の安全に関するものが必要であることが示唆された。
