平成14年度は当初の研究計画に従い,1)文献調査,2)ヒアリング調査,3)理論モデリング,を実施した。1)文献調査では,先進諸国の医療保険制度と規制政策に関する文献を中心にレビューし,ホン研究の対象である保険医療の市場の一部である医療保険市場での購買者としての消費者の役割を強化する政策事例として,西ヨーロッパの社会保険を主体とした保健医療システムを採る国の内で,ドイツ,オランダ,ベルギー,スイスでの,皆保険制度を維持したうえでの消費者による社会保険の選択制度を検討した。これは財源者としての保険者の間に競争を促す政策であり,1990年代の一連の保健医療改革の一環として類似した政策がこれら4ヵ国で採られてきている。さらに,わが国の現行の医療改革政策とこれらの施策の国際比較を試み,その結果を「医療保険制度における消費者機能の応用に関する研究」として平成14年10月に日本公衆衛生学会において発表した。2)文献調査を受けて,12月には国際プロジェクトであるヨーロッパ保健医療政策研究センター(The European Observatory on Health Care Systems)の中核を担っている英国ロンドン大学経済学政治学院に保健・社会ケア部門(LSE Health and Social Care)を訪れ,最新の政策・研究動向をヒアリング調査し,同時に文献等の資料収集を行った。3)理論モデリングに関しては,文献調査の結果に基づいて,医療保険の市場と医療サービスの2つの市場を対象とするモデルを形成するための分析枠組みを検討した。この枠組みは医療経済学,保健医療政策の分野で近年注目されてきている保険者の機能に加えて,新たに消費者の機能の検討を可能にすることを目的としている。これらの成果に基づいて,次年度以降は理論もモデルの開発と実証研究の準備を進めていく方針である。

A series of similar policies for health care reform in the 1990s The results of an international comparison of China's current health care reform policies and policies were presented in a paper entitled "Research on consumer function and use in health care systems" published by the Japan Public Health Association in October 2014. 2)The literature survey was conducted in December to review the latest policy and research trends and to collect data sets from the European Observatory on Health Care Systems. 3)The theory of medical insurance is related to the literature survey results, and the analysis of medical insurance market and medical insurance market is related to the formation of medical insurance market. This group focuses on medical economics, health care policy differentiation, and in recent years, attention has been paid to the function of health care providers, and new consumer functions have been discussed. The results of this research are based on the following year's research and preparation for the development and implementation of the theory and policy.
