2年間の研究を通じて、地域医療情報ネットワーク運用における患者の個人情報保護に際する倫理的ポイントについて、以下の点を明確にした。1.診療目的など、患者に直接利益が還元される1次利用の場合は、Need to Know(知る必要性)の原則に基づいたアクセス・レベルを設定することを前提に、患者取り違え防止の観点からも個人識別情報との分離(=連結不可能匿名化)は行なわない。2.医学研究目的など、患者に直接利益が還元されない2次利用の場合は、原則として個人識別情報と分離し、マッチング不可能な状態(連結不可能匿名化)とし、追跡調査などが不可欠な場合など、個人識別情報とのマッチングなしでは、その研究の意義自体が損なわれてしまう場合に限り、その根拠を明示し、必要性が認められた場合にのみ、識別情報とのマッチングを可能(連結可能匿名化)とする。3.データの2次利用を行なうにあたっては、患者に必ず「連結可能匿名化」とするのか、「連結不可能匿名化」とするのかを明示し、その同意を得ること。連結可能である場合には、どの程度の識別情報との連結が行なわれるのか(氏名、生年月日、現住所、電話番号、本籍、配偶者の有無、職業、家族歴)、また連結の必要性の理由、ならびにその管理方法についても通知し、同意を得ておくこと。4.また「連結可能匿名化」を採る場合には、データの保存期間を提示し、たとえ保存期間内であっても、当該患者に不利益が生じた場合、自らの意思でデータの修正・削除を請求できることを保障する。
