本研究の目的は、がん看護における看護実践の卓越性、および卓越した看護実践の成果を明らかにすることであった。本研究は、予備調査、がん看護に携わる熟練看護師の看護実践に関する調査、およびがん看護専門看護師の看護実践に関する調査の3部構成による、3年間にわたる計画としていた。平成16年度は、平成15年度に引き続き、米国のAdvanced Practice Registered Nurse (APRN)のうち、特にClinical Nurse Specialist (CNS)およびNurse Practitioner (NP)に関する文献を概観し、CNSとNPの活動の現状や課題を検討した。米国においては、州ごとにCNSおよびNPの位置づけが定められ、教育カリキュラムや看護実践範囲、看護実践に対する医療保険償還の規定などが異なっていた。そして、米国におけるAPRNの資格認定制度の統一化が推進されていた。Registered Nurse (RN)と類似して、APRNの看護実践能力には、初心者(novice)から、一人前(competent)、中堅(proficient)エキスパート(expert)までの熟達のレベルがあった。そして、本研究は、CNSとNPの看護実践の卓越性に関する文献的考察に着手した。また、日本におけるがん看護専門看護師に関する文献を概観し、がん看護における看護実践の卓越性に関する文献的考察に着手した。さらに、臨床で活動する看護専門看護師のがん看護における看護実践の卓越性、および卓越した看護実践の成果を明らかにするための現実適合性の高い研究方法論の開発が課題となった。

The purpose of this study is to clarify the excellence and excellence of nursing practice. This study consists of three parts: preliminary survey, survey on nursing practice of skilled nursing staff, survey on nursing practice of skilled nursing staff, and three-year plan. Review of the literature on Advanced Practice Registered Nurse (APRN), Clinical Nurse Specialist (CNS), and Nurse Practitioner (NP) in the United States in 2016 and 2015. China, the state, the CNS, and the NP are located in different areas of the country, including education, nursing, and medical insurance reimbursement. The unification of APRN qualification recognition system in China and the United States has been promoted.
