本研究は家族介護者の意欲を向上させるレスパイトケアのプログラム開発を目的に行った。家族介護者は、レスパイトケアにおいて、レスパイト中の療養者の介護の質を保証することや、療養環境の質を保証すること、それによって療養者の身体面及び精神面の安定を重要なニーズとしてあげていた。特に病院への一時入院によるレスパイトケアを経験した介護者達は、レスパイト後の療養者の身体状態の変化や精神面の動揺を問題点として挙げ、現状では一般的な、病院や施設への一時入院・入所というレスパイト方法に否定的な意見を表していた。一方ケアマネジャーや訪問看護師は、家族の健康状態の維持や介護負担からの一時的緩和を優先されるニーズとしてあげ、レスパイトケアをそのニーズの手段として利用し、家族全体の機能を維持することで、在宅療養継続が得られると捉えていた。人工呼吸器装着の療養者のレスパイトケア目的の一時入院をしている病棟の看護師長へのアンケートでは、療養環境への配慮を最も重要なニードであると捉える一方、ケア度が高く、日常性への配慮が必要であることによるスタッフの負担も大きいと感じていた。これらの結果から、レスパイトケアを促進するためには、療養者に関わる様々な専門職者に対し、役割を分担しながら専門性を発揮することが重要であると共に、一時入院・入所する病院・施設のスタッフへの教育プログラム:(1)対象者への個別ケア、(2)呼吸管理ケア、(3)家族ケアの技術教育を含む事前研修の必要性が示唆された。
