Development of guideline concerning care management that supports cantinuance and mursing prevntion of hame life

1.目的：为了制定支持家庭生活的持续性和护理预防的护理管理指南，阐明了站在老年人方面的护理管理技术.方法：通过研究者制作的访谈记录表和访谈结果，对护理经理专家负责的案例进行了技术上的清晰帮助。结果和结论：从案例分析结果出发，着重对护理管理者帮助技术的特征进行了评价，并将评价项目安排到ICF框架中，制作了评价表。评估表由13个健康状况项目、26个身心功能项目、31个活动项目、10个参与项目、6个环境项目、6个个人因素项目、评估和制定护理计划的过程等8个阶段组成。制定了使用ICF进行评估的指导手册，作为使用评估表进行护理管理评估的指南。指导书被分发给护理经理谁是专家和护理经理谁是专家的医生和物理治疗师谁是专业的康复谁存在于劳动关系的合作，是专家，并获得了评价，并作为一个结果，“指导书的评估使用ICF”作出了进一步的完善。

RESULTS and DISCUSSIONS: It paid attention from the case analysis result to the assessment that the Care manager help technology is characterized, and the assessment table where the assessment item had been arranged to the frame of ICF was made. The assessment table is composed of the step in eight stages as 13 health condition items, 26 mind and body function items, 31 activity items, 10 participation items, 6 environmental items, 6 individual factor items, assessments, and processes of the caring plan making. Guidance book on the assessment that used ICF was made as a guideline of the assessment in care management that used the assessment table. The guidance book was distributed to Care manager who was the expert and Care manager who it was an expert of the doctor and the physical therapist who was the specialty of rehabilitation who existed in the cooperation of labor relation, and was expert and, the evaluation was obtained, and "Guidance book on the assessment that used ICF" was made a refinement further as a result.
