本研究の目的は、「育児支援」の機能を重視した乳幼児健康診査としていくために、各地の乳幼児健康診査の実態および保健師の役割を明らかとし、「育児支援」の機能を取り入れた乳幼児健康診査実施モデルを構築することである。本年度は、全国の乳幼児健康診査における「育児支援」の取り組み状況の実態を調査する。方法は、郵送法による自記式質問紙調査を行った。対象は、全国の保健所409ヵ所の母子保健担当の保健師に対し、管内市町村において、「育児支援」機能を取り入れている乳幼児健康診査を実施している市町村1〜2ヵ所の推薦を依頼した。その後、保健所から推薦のあった市町村および政令市・特別区・中核市383ヵ所の母子保健担当の保健師に対し、質問紙を配布した。調査期間は、平成18年1〜3月である。本研究は、所属研究機関の「疫学倫理委員会」の承認を得た上で実施した。結果、市町村の推薦を依頼した保健所からの回収率は44%であった。推薦のあった市町村、政令市・特別区・中核市からの回収率は36%(現在、回収中)であった。具体的な育児支援の工夫として、「乳幼児健康診査実施中の工夫」は、問診票の工夫(虐待予防スクリーニングの活用、育児姿勢・育児不安・心のヘルスチェックの把握)、音楽の活用、絵本の読み聞かせ、スタッフ構成や方法の充実、母親の友達づくりの機会、離乳食・幼児教室の同時開催などであった。「乳幼児健康診査実施後の工夫」は、健診の満足度を聴取し声を繁栄した健診体制の整備、未受診者への育児支援家庭訪問の実施などであった。「乳幼児健康診査の体制整備の工夫」は、妊娠時から就学までの一貫した体制、健診と中高生の赤ちゃん体験との同時開催による虐待予防と思春期保健の推進、児童虐待予防対策委員会などの設置による関係機関との連携などであった。全国各地における創意工夫のある「育児支援」機能を取り入れた乳幼児健康診査の実態が明らかとなった。
