本研究は、介護保険制度下におけるケアマネジメントにおいて、対応困難を生じている介護支援専門員に対する効果的な支援技術を構築することを目的としている。平成19年度は、今までに行った保健師へのインタビュー調査及び介護支援専門員に対する調査結果を分析し、対応困難を生じている介護支援専門員への支援行為として、『介護支援専門員に向けた行為』13カテゴリ(「問題を整理する」、「援助方針を示す」、「対応方法を示す」等)、『利用者・家族に向けた行為』9カテゴリ(「利用者の状況を確認する」、「家族調整をする」等)、『ケアチームに向けた行為』6カテゴリ(「ケアチームの協議の場を設定する」、「ケアチームの支援結果を確認する」等)、『ケアスタッフ・関係者に向けた行為』8カテゴリ(「関係者に情報提供を依頼する」、「行政的な仕組みを作る」等)を抽出した。そして、この結果をもとに介護支援専門員に対する支援活動指標(案)を作成した。その内容を現在N県及びG県の地域包括支援センター及び行政の介護保険部門に勤務し、介護支援専門員への支援経験のある保健師5名に対しその活用可能性について聴取した。また、自記式質問紙を作成し、全国の地域包括支援センター協議会及びN県・関東地域の地域包括支援センターから150か所を無作為抽出し、アンケート調査を行った(有効回答率56%)。調査結果から、活動項目について、概ねどの項目においても、7〜8割以上の人が介護支援専門員への支援行為として「重要である」と回答したが、実際の実施経験の有無にはばらつきが見られた。これらのことから、本活動指標は、対応困難を生じている介護支援専門員への支援活動を行う上で重要な要素を含んでおり、その活用可能性が示された。今後、さらに内容を精選していくとともに、実践現場で活用できるよう働きかけていく必要がある。

This study aims to establish support technologies for the development of health care services under the health care system. In addition, the survey was conducted in the regions of the country, including the support of the national agreement and the Kanto region, with a response rate of 56%. Survey results include: activity items, summary items, support behaviors of mediating support personnel for people with more than 7 - 8 cuts,"important" answers, actual implementation of management and presence or absence of support activities. This activity indicator shows the possibility of using the important elements of the support activities of the mediating support personnel in response to difficulties. In the future, the content of this article will be selected and used in practice.
