项目摘要

近年の国立大学の独立行政法人化に伴い、大学法人は独立採算性を求められるようになった。大学附属病院においてもそれは例外ではなく、費用対効果を考慮した運営が必要とされつつある。しかし現時点において、効果、すなわち業績をどのように評価するのかという確固たる指標が存在しないという問題に直面する。米国においては営利・非営利病院の役割分担が明確にされており、そのため厳格な業績評価指標が存在する。こうした米国における業績評価基準は、これから業績評価を導入する日本の医療機関からすると大いに参考になると考えられる。しかしながらその指標は病院ごとの采配に委ねられており、詳細なヒアリング調査による情報収集が不可欠と言える。更に教育機関としてのあり方とコスト管理という観点から、医師、特に指導医と研修医の人数バランスや処遇面といった具体的な人事の手法についても学ばねばならない点が多いと考える。大学運営、病院経営、そして医学教育というそれぞれの側面からの情報収集・分析を行い、日本の大学病院運営に合致したシステム構築を調査するため、日米文化センター主催の「医療経営革新のための訪米研修プログラム」に参加した。12カ所に及ぶ訪問調査やインタビュー結果を現在まとめており、当院を含め赤字経営の続く国立大学附属病院や自治体病院に、黒字転換への経営戦略と充実した教育を提供する事の出来る「人気のある病院」足り得る指標を掲げる論文を提出予定である。
