项目摘要

<フィールド調査>[目的]介護老人福祉施設の経管栄養利用者に対する施設方針、事故状況を把握する。[方法]東京都内の2施設の施設長・看護主任へのインタビュー調査を行った。[結果]1介護老人福祉施設の経管栄養利用者(全て胃瘻)は6人であった。施設では「入居者が入院先で経管栄養の導入がなされた場合のみ受け入れる」方針であった。他の1介護老人福祉施設は「経口摂取が原則であり経管栄養利用者の入居は受け入れない」方針であった。この施設では入院で胃瘻導入された1人に対し家族に対する経管栄養の指導を施設内で行い自宅への退所を支援した。事故状況では自己抜去や、自己抜去予防の抑制の問題が指摘された。[考察]対象施設では入居者の入院に伴う経管栄養導入は受け入れていたが、経管栄養利用者の新規入居には制限をかけていた。<全国調査>[目的]全国の介護老人福祉施設の経管栄養利用者割合と今後の受け入れ状況を明らかにする。[方法]全国介護老人福祉施設調査(都道府県単位25%無作為抽出1,437施設)に郵送自記式調査を行った。[結果]経管栄養利用入居者数に回答のあった897施設のうち82施設(9.1%)が経管栄養利用者がいないと回答した。現在の対施設利用者経管栄養利用者割合は中央値8.0(範囲0〜100)%であった。対定員の現在の経管栄養利用者数に今後の受け入れ可能利用者数を加えた値の中央値は13.3(0-100)%であった。自由記載では「入居者が入院後経管栄養で戻ってくるため新規入所は受け入れられない」「あまりにも入退院を繰り返す状況があり対応困難」等の記載があった。[考察]現在の介護老人福祉施設の経管栄養利用者数は1割未満であり、施設受け入れが可能と考える数を加えても定員の2割を超えないことが明らかになった。今後、頻回な入退院の問題や、吸引処置への対応について十分な対策が必要と推測された。
