【目的】薬物療法における副作用回避や適切な薬物選択は患者利益に貢献すると考えられる。本研究では、統合失調症患者における非定型抗精神病薬4剤(Risperidone, Quetiapine, Olanzapine, Perospirone)の治療反応性について精神症状評価尺度(BPRS: Brief Psychotic Rating Scale、 CGI: Clinical Global Impression)を用い薬剤とDRD2多型(-141de1C,TaqIA,Ser311Cys)との関係を検討した。【方法】治療前にBPRSを施行、単剤治療を6週間行った後にBPRSおよびCGIを行った。遺伝子解析は血液検体から直接シークエンス法にて行い、統計解析はBPRS20%以上の改善あるいはBPRS改善度15%以上かつCGI軽度改善以上のものを反応群と定義し、4剤それぞれの反応群と遺伝子型(ゲノタイプ)およびアレル型頻度の解析にχ^2検定を施行した。本研究は金沢医科大学遺伝子倫理委員会から承認を得ており、全対象者から文書による同意を得ている。【結果および考察】risperidone26例、quetiapine5例、olanzapine6例、perospirone10例の計47例について統計解析を行ったが、いずれの薬剤間においても有意な関係性は認めなかった(-141de1C:ゲノタイプ;p=0.400〜1.000、アレル;P=0.545〜1.000、TaqIA:ゲノタイプ;p=0.202〜1.000、アレル;P=0.240〜1.000、Ser311Cys:1例を除いてSerのみであったので未解析)。過去には各々背景は異なるが有意な関連性が見られなかった報告が多く、一定の結果は得られていない。本研究では対象例数が少ないが、今後多くの母集団での研究を行い、個別化治療確立に貢献したい。
