项目摘要

1.アメリカ社会で新たな問題となっている「肥満」という現象を分析対象として、以下の三つに焦点を絞り、調査地をカリフォルニア州ベイエリア地区として、身体を持つ個々人が直面すると生の問題に取り組んだ。(1)低所得者層向けの栄養カウンセリングを目的とした政府のプログラムを参与観察した。同時に、公衆衛生の人々(研究所・政府管轄団体・NPO団体など)にたいし、肥満予防への取り組みについてインタビュー調査を実施。(2)他方で、低所得者層以外の肥満も増加していることは事実で、これについては公衆衛生の分野ではほとんど議論の対象になっていない。そこで、ベストセラーになっているダイエット本20冊の比較調査、減量クリニックと減量プログラムでの参与観察を実施することによって、個々人がどのように減量に取り組んでいるのかを調査した。(3)保健省などによって、長期にわたる食行動の改善により減量は可能であるという呼びかけが中心である一方、その方法についての合意は未だないため、多くの肥満者は社会的ポリティクスとスティグマに葛藤している。そこで、肥満手術者、肥満受容運動をしている人々への聞き取り調査と参与観察を実施。2.結果と今後の発展保健医療政策では、エスニック・マイノリティや低所得者層に生じる健康格差が肥満問題の克服すべき中心課題とし、貧困地区などの「コミュニティ」の再創造に向けた政策を打ち出す。その一方で、政策を通じて人々はリスク集団群に分割され、公衆衛生側としても、結局、個々人に任せるという図式が成り立ってしまい、コミュニティ活動としての連帯には結びつきにくい。また、ダイエットに失敗し続け「太りすぎ」てしまった人々は、肥満は意志の問題ではなく、遺伝的要素が強いと結論付ける傾向が強い。今後の発展性としては、体重を健康の指標とせずに健康作りを目指していこうとする、Health at Every Sizeという新たな運動に焦点を当てて、研究をさらに発展させていく。
