ギアチェンジを"抗がん治療をしている患者が、治療の目的を治癒以外の方向に転換していくこと"と捉え、医療者のがん患者のギアチェンジに対する認識、ギアチェンジを支える援助内容と援助を提供する上での阻害要因を明らかにした。看護師はがん患者のギアチェンジに対して課題、意義、実現するための看護の役割を捉える一方、心的負担を認識しつつ援助を行っていた。終末期がん医療は不確かで、ギアチェンジを支える援助に確信が持ちにくいという特徴が示された。
