项目摘要

１．子ども虐待予防のための個別支援とグループ支援（MCG：Mother and Child Group）の併用による包括的な支援プログラムとその効果を検討するため、支援経験のある保健師を対象に質的調査を実施した。今年度は保健師が母親をグループにつなぎ参加を継続するためにどのような支援をしているのか検討した。２．対象：保健センター等計7か所の虐待予防のグループ支援に関わった経験がある保健師17名を対象に1時間程度の半構成的インタビュー調査を実施した。３．内容：背景、母親の特徴とグループ参加状況、支援と課題、参加継続の支援と体制、グループ支援と個別支援の関係、参加により期待される効果、等とした。1時間程度の半構成的インタビュー調査を実施し、分析は情報をコード化、カテゴリー化し、内容分析を実施した。４．結果：保健師の経験年数は平均13.0年、グループ支援経験年数は平均3.1年であった。グループ参加を促す支援として、1)前提となる支援：「どの対象にどのタイミングでグループに誘う声掛けをするか考える」「参加の目的を母親と一緒に確認し合う」「保健師自身がグループに参加した経験があるとつなげやすい」、2)グループの理解を促す支援：「お母さん同士が育児から少し離れて、自分の思いを話せる場所を作っている」「子どもは保育が確保されているので自分のことを語っていい場」「秘密も守られる」「他の人の話も聞けて自分の整理にもなる」「話したくなければ言わなくてもいいし、守られた環境で気持ちを吐き出すことができる」「子育てに不安や育児に自信がない方を紹介していると伝えると、自分も心配な人なのだというイメージを与えてしまうためポジティブな誘い方をしている」、3)参加後の継続を支える支援：「参加してどうだったか一緒に振り返る」「グループにつなげたまま放置しない」など個別支援の重要性が明らかになった。
