Mechanisms of ischemia-induced excessive excitement of cardiac autonomic nerves and the influence of this excitement to myocardial injury

本研究では、心臓マイクロダイアリシス法を用い、虚血・再灌流時における交感・副交感神経傷害の発生機序、自律神経傷害の心筋細胞傷害に及ぼす影響、ならびpre-conditioningが自律神経傷害ならびに心筋細胞傷害を軽減するかを検討した。虚血・再灌流時の虚血部心臓交感・副交感神経傷害に関して、心虚血時に虚血部交感神経終末から非開口分泌によりnorepinephrine(NE)が大量に放出されるが、その一部は、心臓内PNMTによりepinephrine(Epi)に変換された。また、心虚血時の虚血部交感・副交感神経終末からのNEおよびAch放出は、神経活動とは関係なく放出されるが、その機序として、虚血時に起こるNa^+-K^+ ATPaseのブロックが関わっていた。さらに、心虚血時の虚血部交感・副交感神経終末からのNEおよびAchの放出の程度は、虚血の程度に深く関わっていた。虚血・再灌流時の虚血部心筋細胞傷害に関して、心虚血時に虚血部交感神経終末からの非開口分泌によってNE が放出されるが、それを抑制しても、虚血・再灌流時の心筋細胞傷害に影響はなかった。また、迷走神経電気刺激により心虚血・再灌流時の心筋細胞傷害は減弱したが、これは心拍数低下にともなう心筋酸素消費量の低下によるところが大きかった。さらに、虚血前の吸入麻酔薬sevoflurane投与は、心臓に直接作用して虚血・再灌流時の心筋細胞傷害を軽減した。
This study investigated the effects of cardiac arrest, the development of sympathetic and parasympathetic neurologic injury, autonomic neurologic injury and cardiac muscle cell injury, and pre-conditioning on the reduction of autonomic neurologic injury and cardiac muscle cell injury during ischemia and reperfusion. During reperfusion, the sympathetic and parasympathetic nervous system of the deficient part of the blood is damaged, and during heart failure, the sympathetic nervous system of the deficient part of the blood is released in large quantities, and the PNMT (Epi) is replaced. In case of deficiency of blood, sympathetic and parasympathetic nervous system terminal changes, NE and Ach release changes, nervous system activity changes, mechanism changes, Na^+-K^+ ATPase changes, and so on. When the heart is weak, the blood is weak, the sympathetic and parasympathetic nervous system ends, and the degree of Ach release is deeply related to the degree of blood deficiency. The heart muscle cell injury in the deficient blood part during reperfusion is related to the end of sympathetic nervous system in the deficient blood part, and the NE secretion is inhibited during reperfusion. The heart muscle cell injury in the deficient blood part is affected by the end of sympathetic nervous system in the deficient blood part. The heart muscle cell damage during reperfusion was reduced due to the decrease in cardiac beats and the decrease in cardiac muscle acid consumption. Inhalation of sevoflurane prior to ischemia reduces injury to cardiac muscle cells during ischemia and reperfusion.
