项目摘要

近年、介護老人保健施設(以下、老健と示す)でも終末期を迎える入所者が増加している。しかし終末期ケアの判断基準とケアの方針が未確立であることが指摘されており、終末期ケアの内容は施設の裁量によるところが大きいという現状がある。そこで老健における終末期ケアの質を保証するためにも、終末期ケアの質を評価する指標が必要であると考えた。その第1段階として平成22年度に、文献検討から作成した老健における終末期ケアの質評価指標案の適切性および重要性について検討した。平成23年度は、第1段階で作成した質評価指標案の実施可能性と選定した項目の妥当性を検討した。調査方法は自記式質問紙法で、対象は終末期ケアを実施している老健1,000施設を無作為に抽出し、看護主任または老健において経験豊富な看護職1施設1名に回答を依頼した。質問内容は、第1段階で作成した指標項目についての実施状況(1=全くしていない～5=必ずしている)と重要性の認識(1=あまり重要でない～5=非常に重要である)について尋ねた。さらに指標項目の妥当性を検討する目的で施設の管理体制11項目について構造評価を行った。その結果、指標項目の重要性は71個項目全ての平均値が4以上であり「重要である」と認識されていた。実施状況は34項目の平均値が4以上で項目のケア内容を「おおむね」実施していた。また選定された指標項目の妥当性を検討するために、終末期ケアの実施状況71項目の合計得点と施設の管理体制で「はい」と回答した項目数についてSpearmanの順位相関係数を算出した結果、0.52(p<0.01)であった。全指標項目が「重要である」と認識されており、施設の管理体制が整備されているほどケア項目の実施率も高かったため妥当な項目が選定できたと考えられた。

The purpose of the evaluation of the appropriateness of the index item is to establish a management system for 11 items. The results, the importance of the indicator items, the average value of the 71 items, and the above are recognized as important. The average value of 34 items is more than 4, and the content of the item is more than 4. The appropriateness of the selected indicator items was discussed, and the implementation status of the final stage was 71. The total score of the items and the management system of the implementation were calculated. The number of items was 0.52(p<0.01).
