従来の研究では、介護負担感の関連要因として、高齢者の認知機能や日常生活動作能力(Activity of Daily Living)の低下、認知症による問題行動といった要介護者の要因に加え、介護者の年齢や、経済状態が低いことが報告されているが、これらの要因への介入は難しいために、負担軽減のための十分な支援に繋がっていないのが現状である。本研究では、直接の支援につなげることが可能な要因の一つとして地域の支え合いやネットワークといった地域環境要因に注目し、これらが介護者の精神健康に与える影響について検討を行うことを目的とした。本研究結果から、地域支援のあり方について提言を行うことができると期待される。2010年3月～6月、A県の協力を得て、県内すべての15介護保険者と1介護保険広域連合の要介護認定者リストから、要介護1～5の認定を受けている第1号被保険者のうち、無作為に抽出された5639名を対象に、郵送による自記式質問紙調査を実施した。調査内容は、介護の負担、抑うつの程度、地域環境要因としての介護のしやすさ、地域のネットワーク、さらに、介護状況、介護保険サービスの利用状況等とした。回収率は51.4%で、最終分析対象は2511票であった。平均年齢は63歳、女性介護者75.4%、4割を超える介護者が65歳以上という老老介護の現状にあること、4割近くの介護者が「仕事」と「家族の介護」の両方を担っていること、認知症状を有する要介護者の介護者は7割近くいることが明らかになった。加えて、介護者自身に健康不安を抱えているものも4割近くおり、非常に脆弱な介護者の状況が明らかになった。また、地域環境要因と精神健康との関連では、地域環境要因の指標の一つである地域に対する信頼と互酬性を持つ介護者の方が、介護の負担や抑うつの程度は低く、地域の環境が介護者の精神健康に影響していることが明らかになった。今後は、本結果を基に地域支援に向けた具体的施策についての検討を行う予定である。
