项目摘要

1.日本人集団におけるインスリン分泌不全と抵抗性の2型糖尿病発症への影響の明確化喫煙のインスリン分泌不全と抵抗性への影響を検討するにあたり、その前に、日本人集団におけるインスリン分泌不全と抵抗性の2型糖尿病発症への影響を検討した。その結果、正常者に比べてインスリン分泌不全単独者で7.61倍(95%信頼区間:4.89-11.84)、インスリン抵抗性単独者で4.70倍(95%信頼区間:2.81-7.88)、インスリン分泌不全と抵抗性両方ともある者で16.06倍(95%信頼区間:9.22-28.00)、2型糖尿病発症リスクが上昇した。加えて、ベースライン時にインスリン分泌不全の者が多く、そのため2型糖尿病発症に対する集団リスク(人口寄与危険度割合)は、インスリン分泌不全単独者で49.0%(95%信頼区間:44.8-51.6)、インスリン抵抗性単独者で14.3%(95%信頼区間:11.7-15.8)、インスリン分泌不全と抵抗性両方ともある者で12.9%(95%信頼区間:12.2-13.2)であった。このことより、日本人集団においては2型糖尿病発症にはインスリン分泌不全の影響が大きいことが明らかになった。2.喫煙のインスリン分泌不全と抵抗性それぞれへの影響の明確化糖尿病、インスリン分泌不全、インスリン抵抗性がない者において検討した結果、非喫煙者に比べて喫煙者は1.95倍(95%信頼区間:1.44-2.63)、インスリン分泌不全になるリスクが上昇した。加えて、1日の喫煙本数と喫煙年数から算出したパック年はインスリン分泌不全になるリスクと量反応関係があった(Pfortrend<0.001)。一方、喫煙はインスリン抵抗性リスクに関連していなかった。このことより、喫煙は2型糖尿病発症への影響が大きいインスリン分泌不全の修正可能な危険因子であることが明らかになった。

1. The Japanese Group discusses the effects of smoking on the development of type 2 diabetes mellitus and resistance to diabetes mellitus.
