项目摘要

本年度の研究では，1)取引慣行の流通価格（薬価）に対する影響に関する実取引データを用いた実証分析，2）取引慣行の改善に関する政策シミュレーションを行ってきた．加えて，3）医薬品需要関数の推定による医薬分業の政策評価を行ってきた．以上の成果は2本の学術論文と筆者の博士論文にまとめられている．まず，1)の研究では，医薬品卸企業から提供された医薬品流通に関する実取引データを用いて，製薬企業が卸企業に支払う後補償としてのリベート・アローアンスや価格妥結時期が，仕切価格及び納入価格にどのような影響を与えているのか推定した．推定では，医療機関ごとの異質性や分析上生じる内生性の問題に考慮した推定を行った．この手法はInternational Journal of Health Policy and Managementに掲載予定の “Does Scale of Public Hospitals Affect Bargaining Power? Evidence From Japan”, (with Takao Kojo and Ichiro Innami)と同様の手法を用いている．次に，2)については，複数の薬価データから近似した仮想的医薬品の流通価格を各取引慣行が存在する医薬品流通取引モデルに適用し，取引慣行の改善（縮小）を想定した政策シミュレーションを行った．この成果は「医薬品流通における取引慣行改善のモデルシミュレーション」(印南一路との共著)として『公共選択』に掲載予定である．3）については，医薬品流通の実取引データを用いて，薬価差益に対する医療機関別の医薬品需要関数を推定し，80年から90年代の推定値と比較検討を行った．本研究は博士論文「医療用医薬品における取引慣行と流通価格-実取引データを用いた実証分析と政策シミュレーション-」の補論として掲載される．

This year's research includes: 1) empirical analysis of the impact of selected practices on circulation patterns; 2) policy formulation and implementation of selected practices on improvement. 3) The estimation of the number of medical drug needs and the evaluation of the policy of the medical industry. The above achievements are two academic papers and the author's doctoral thesis. In this study, 1) medical product manufacturers provide medical product circulation related to the actual use of medical products, manufacturing enterprises to unload enterprises after the compensation, the loss of medical products, the completion of the appropriate period, the status and the inclusion of medical products related to the impact and estimation of medical products. Presumption, heterogeneity of medical institutions, analysis of endogenous problems, consideration of presumptions. Second, 2) in the middle of the process, a plurality of drug distribution patterns approximate to the desired drug distribution pattern, and each of the drug distribution patterns has a habit of existing, and the drug distribution pattern has a habit of improving (narrowing) the desired policy. The results of this study include the following: "A study on the improvement of medical drug circulation practices"(co-authored by the Indian and South China Railway) and "Public Choice"(published in 2003). This study is a supplement to the doctoral thesis "medical drug selection, circulation, research and application, evidence-based analysis and policy development".
