がんと診断された時から終末期にいたるまで、患者QOLの向上や患者の意向に沿った治療やケアを提供するために、患者自身の評価や症状の訴え：患者報告型アウトカムの利用が重要視されている。しかし、臨床現場における効果的かつ効率的な利用方法については未だ明確になっておらず、医療者は患者の症状や直面する問題を正確に評価できていないのが現状である。本研究の目的は、患者報告型アウトカムの特異的な現象として健康の変化によって個人内の判断基準が変化する現象：レスポンスシフトを考慮した上で外来がん患者における最適な薬学的支援方法を構築し、また在宅緩和ケアを受けている終末期患者の症状クラスターの特徴を明らかにすることでより包括的な緩和ケアの方法を構築することである。本研究の目的は、1）外来がん患者においては薬学的支援方法の介入効果とレスポンスシフトの検討を行い、2）在宅患者においては緩和ケア患者の症状クラスターの調査と予後や患者QOLとの関係性の検討を実施予定であった。初年度は在宅患者における苦痛症状における程度を明らかにすることを目的に緩和医療の対象となる患者が頻繁に経験する9つの症状（痛み、だるさ、眠気、吐き気、食欲不振、息苦しさ、気分の落ち込み、不安、全体的な調子）について評価可能なエドモントン症状評価システム改訂版（日本語版） (Edmonton Symptom Assessment System Revised Japanese version: ESAS-r-J) を使用する患者報告型アウトカムを利用したアンケート調査を実施する予定であったが、十分な症例数を集めるに至らなかった。

The most appropriate learning support method for patients in the terminal stage is to consider the characteristics of patients in the terminal stage, including the method of mitigation. The objectives of this study are: 1) to investigate the intervention results of support methods for external patients; 2) to investigate the symptoms of internal patients; and 3) to determine the relationship between QOL and external patients. In the beginning of the year, the patient suffered from pain and symptoms, and the patient suffered from frequent symptoms.(Pain, sleep, vomiting, loss of appetite, pain, depression, restlessness, general mood)(Edmonton Symptom Assessment System Revised Japanese version: ESAS-r-J) To use the patient report type, use the patient report type to conduct the survey.
