项目摘要

本研究は，障害のある子どもたちが安全で美味しい学校給食を通して適切に摂食機能の学習ができるように，日常生活を支える教員と共に摂食機能の阻害因子を的確に判断できる医療専門職との円滑な医教連携の実現にむけた取り組みのひとつである。具体的には，4年間で，地域格差なく教育職と医療職が知識や困りごとを共有・解決できるWEBページやアプリケーション（アプリ）などの開発を行うことを目標にしている。そこで初年度である本年度は全国の特別支援学校において学校給食の現状，学校給食で困っている内容を把握し，必要な情報提供の内容について検討を行うための予備調査として，某県の全特別支援学校に対して給食指導などについて無記名の郵送法調査を行った。調査項目は，学校給食の現状，医療職による摂食指導の有無，関わる医療職，誤嚥・窒息のリスクの多い医療的ケアを必要とする児童生徒に関して学校給食で行っている特別な配慮や給食提供内容とした。37%の学校から回収ができ，そのうち研究協力を得られた88％について以下に結果を示す。医療的ケアが必要な児童生徒が在籍している学校は88％で，医療職による給食指導の実施校は69%であった。経管栄養の児童生徒に学校給食を提供している学校は62%，学校給食をペースト状にした食事を胃瘻から注入している胃ろう食を提供している学校は38%に認められた。本研究結果から，重度の摂食機能障害を伴う児童生徒であっても学校給食が提供され，以前から課題となっていた学校給食の胃瘻からの注入が一部の学校では行われるようになってきた傾向が理解された。それらの結果から，教員が学校にて医療的ケアを必要とする児童生徒の食事介助や摂食指導を行う機会が増えると予想される。来年度に実施する全国の特別支援学校に向けた調査においては医療的ケア児への学校給食に対する不安項目なども含めて作成する必要が示唆された。
