Patient-reported outcome measures (PROMs) have the potential to empower and engage patients with their care, particularly in the days-to-weeks following discharge from hospital. Hospital care can leave people feeling disoriented and unsure of the advice they may have been given during their hospital stay. Tasking patients with measuring a set of symptoms can be a helpful way to empower patients with more confidence in their recovery, or can act as an early warning to deterioration.Unfortunately, existing PROM solutions struggle to gain uptake as they often inadvertently either create more work for clinicians (who have to review these measures), or little value for patients (as measures cannot be reviewed and acted on).Our solution is for PROMs to be driven by clinical tasks. By replacing measures that clinicians are spending time taking (such as the feeding patterns of a newborn), not only could we empower patients with confidence that, in this example, their newborn is feeding well, but we would also be reducing the burden on clinicians.Clinical Task Systems Ltd (CTS), founded together by an NHS midwife and an NHS junior doctor are developing the Lister task management platform and are therefore uniquely placed to develop the proposed PROMs system by linking PROMs to and from underlying clinical tasks. Only such an integrated approach has hope for unlocking the value of PROMs for supporting post-discharge care and reducing readmissions to hospital.In this particular project, we aim to establish the feasibility of our proposed approach. We do this by focusing on mothers and babies discharged after giving birth - a time which is undoubtedly very disorienting and challenging. Proving feasibility and value in this specific instance will enable us to start building a far more dynamic PROMs system, which will leverage artificial intelligence to generate the right PROMs for the right patients, based on the underlying tasks that their clinical team are trying to accomplish.Our solution will enable more collaborative and patient-centred care to flourish.
