HORMONAL,VISUAL & NEUROPSYCHOLOGIC DYSFUNCTION IN PITUITARY HYPOTHALAMIC DISEASE

本项目的目的是更好地确定肿瘤、脑外伤或其他涉及垂体和下丘脑的病理患者的激素、视觉和神经心理功能障碍(S)的发生率、解剖相关性、病理生理学、自然病史或治疗反应。每名患者在手术、放射或内科治疗前、后每隔6-24个月进行一系列脑垂体前、后叶、视觉、神经心理和核磁共振检查。治疗的类型将由患者被转介到的医生选择。功能测试的结果将与其他变量一起分析，以确定1)存在的功能障碍的发生率和类型；2)它们与MRI上病变位置的相关性；3)它们与随后的激素、视觉或神经心理功能的临床障碍的关系；以及4)它们在不同类型的治疗后随时间的变化。我们预计，概述的信息将改善各种类型的垂体下丘脑疾病患者的管理，并可能在确定调节这些重要活动的神经系统的位置和/或功能组织方面具有基本价值。

The purpose of this project is to better define the incidence, anatomical correlates, pathophysiology, natural history, or response to treatment of hormonal, visual and neuropsychologic dysfunction(s) in patients with tumors, head trauma or other pathology involving the pituitary and hypothalamus. In each patient, a battery of anterior pituitary, posterior pituitary, visual, neuropsychologic and MRI tests will be given before and at 6-24 month intervals after surgical, radiologic or medical therapy. The type of therapy will be chosen by the physician to whom the patient was referred. The results of the functional tests will be analyzed in conjunction with the other variables to determine 1) the incidence and types of dysfunction present; 2) their correlation if any with the location of the lesion on MRI; 3) their relationship to subsequent clinical disturbances of hormonal, visual or neuropsychologic function; and 4) their change over time following different kinds of treatment. We anticipate that the information outlined will improve the management of patients with various types of pituitary hypothalamic disease, and may also be of fundamental value in determining the location and/or functional organization of the neuronal systems that regulate these vital activities.
