EXCEED THE SPACE PROVIDED. The rapid progression of scientific discovery, particularly at the cellular and molecular levels, offers unprecedented opportunities for the direct application of basic scientific findings to gastrointestinal disease. Such progress can be optimized by the provision of rigorous training in basic scientific thought, as well as technique, to a few young surgeons who are already directed towards academic leadership. The specific aim of this program is to provide formal, basic scientific training to this select group, as an integral part of their post graduate clinical training. One trainee is selected competitively each year from those applying for residency training in GI Surgery at Johns Hopkins. During the first of three post-graduate years of clinical training he/she is guided to choose a senior investigator/preceptor who has demonstrated an outstanding record of independent basic scientific achievement, as well as an established record of mentorship. Most mentors are basic scientists. The formal scientific training period will begin after the third clinical year, and last for three consecutive years, uninterrupted by clinical duties. Trainees who begin the program already holding a PhD, as well as an MD degree, receive three additional years of intensive post-doctoral training with course work added only to supplement particular weaknesses or to facilitate expanded capabilities. Trainees holding only a MD degree receive an initial year of course work and labatory experience followed by two additional years of closely supervised laboratory research often leading to a PhD degree in the Cellular and Molecular Medicine Program or its equivalent. The preceptor will maintain the mentor relationship for the entire nine years of research and clinical training, and in most cases, an additional final year of research, funded by a level-appropriate grant (eg: K08). Thus the trainee should be able to start his/her initial faculty position, at JHU or elsewhere, not only well-trained, but funded. This program is designed, in this way, to generate a very small, select group of GI surgeon-scientists to provide academic scientific leadership for the 21st century.
