REVISING ACR DIAGNOSTIC/CLASSIFICATION CRITERIA FOR LUPUS

This study seeks to develop a new classification criteria decided by under the American College of Rheumatology (heretofore referred to as ACR), for the diagnosis and classification of Systemic Lupus Erythematosus (heretofore referred to as SLE), commonly referred to as "Lupus." Currently used is classification criteria that was originally published in 1971, revised in 1982 and 1996. Research and clinical observation has shown these criteria to be insufficient in the diagnosis and classification of SLE. There is concern that the current criteria does not allow for a thorough representation of the Lupus population. Many patients go undiagnosed because of the limitations allowed for by the current criteria. Specific Aims of this multicenter design: To develop a classification criteria for systemic lupus erythematosus by: 1. Collection of criteria in 600 SLE cases and disease controls (with identical laboratory assays in all) 2. To include in the dataset only those cases and controls for which there is agreement on the diagnosis by a consensus panel of rotating rheumatologists, dermatologists, and nephrologists. 3. To develop, by an iterative approach that includes review of each statistical model by the participating physicians, the classification criteria, using multiple methods, including classification trees, logistic regression, and weighting of criteria. 4. To validate the classification criteria using: 1. 100 SLE cases and controls set aside from the original 600; 2. additional patients from the participating centers.
