AEROBIC PERFORMANCE, MOBILITY LIMITATIONS AND EXERCISE IN OLDER ADULTS WITH DIAB

This subproject is one of many research subprojects utilizing the resources provided by a Center grant funded by NIH/NCRR. The subproject and investigator (PI) may have received primary funding from another NIH source, and thus could be represented in other CRISP entries. The institution listed is for the Center, which is not necessarily the institution for the investigator. The purpose of this study is to conduct a randomized controlled trial in 240 older adults with diabetes to determine the effects of functional circuit training and activity tailored intervention/behavioral support (with a home program of behavioral support) or flexibility controls and a program of health education and no behavioral support) on measures of peak aerobic capacity, submaximal oxygen kinetics, functional mobility performance and self-reported function. Both sexes will be recruited. We will particularly encourage African Americans in our recruitment, with a goal of attaining a distribution similar to that of older diabetics. Other minorities will be recruited based on their percentage in Washtenaw county and western Wayne county, the primary recruitment region (3% Asian American, and 2% Hispanic). No vulnerable (such as demented) populations will be involved. We propose to test the hypothesis that aerobic performance is associated with mobility limitations, and that an intervention to improve aerobic performance will also improve mobility. Given the low physical activity levels of many older adults with diabetes we will specifically test whether exercise interventions that improve submaximal aerobic performance also improve mobility. The majority of data collected will be directly from the human subject, in the form of questionnaire responses, actual observed performance, blood tests, DEXA scan and echocardiogram. We will also request to corroborate the subject interview with inspection of their medical records which can include information about their number of physician visits and hospitalizations related to diabetes. All HIPAA requirements will be carefully followed. Confidentiality will be maintained to the fullest extent possible. Longstanding protocols have been in place, including data coding by subject number, locked in file cabinets, and cross-referenced by a name-number key in another location. A comprehensive written consent process will be employed.
