This subproject is one of many research subprojects utilizing the resources provided by a Center grant funded by NIH/NCRR. The subproject and investigator (PI) may have received primary funding from another NIH source, and thus could be represented in other CRISP entries. The institution listed is for the Center, which is not necessarily the institution for the investigator. Nitric oxide is a natural gas ordinarily produced in the lung, which opens the blood vessels going to the lungs. The effect of breathing small amounts of nitric oxide gas has been studied in adults and infants with other types of diseases affecting blood flow to the lungs. The tests done so far suggest that nitric oxide can make it easier for blood to flow to the lungs and can improve blood flow to the healthiest parts of the lung. Nitric oxide has been used in a small number of premature infants with severe CLD who are on ventilators. Some infants with CLD who have received nitric oxide so far have demonstrated a decrease in the amount of oxygen that they require, and over time they have been able to tolerate lower ventilator settings. It is important to understand that high ventilator pressures and high oxygen are known to contribute to lung injury. The purpose of this study is to determine whether premature babies who still need to be on the ventilator at 2 weeks of life will be less likely to develop CLD if they receive a small amount of nitric oxide through the ventilator.
