This subproject is one of many research subprojects utilizing the
resources provided by a Center grant funded by NIH/NCRR. The subproject and
investigator (PI) may have received primary funding from another NIH source,
and thus could be represented in other CRISP entries. The institution listed is
for the Center, which is not necessarily the institution for the investigator.
Obesity is increasing dramatically in young people, and with it the incidence of type 2 diabetes. This trend is especially pronounced among African Americans. About 16% of young people with new onset diabetes are found to have type 2 diabetes; among African-American youth in Chicago the proportion is 28%.
As obesity is the major modifiable risk factor for type 2 diabetes, this diabetes prevention trial focuses on decreasing obesity (promoting weight loss/preventing weight gain, increasing activity, and changing eating patterns) in order to decrease risk for type 2 diabetes.
Study Question: Can a culturally sensitive, community-based, family oriented intervention to decrease obesity and risk of type 2 diabetes in a high risk, urban, African-American population succeed?
Design: We will recruit a total of 190 African American families (29 in pilot) known to be at high risk for diabetes. Families are randomized to high intensity behavioral intervention or standard care (control) groups. The intervention group participates in 14 weekly nutrition and exercise sessions. Subjects are followed for clinical outcomes for two years.
Study Population: The study population is overweight (>85% BMI z-score) African American children between the ages of 9-12 with a family history of type 2 diabetes and accompanied by a parent/guardian.
Primary outcome measure include BMI z-score, BMI (for adults), fasting glucose to insulin ratio, area under the curve (2 hour GTT), and body fat. Secondary outcomes include nutrition habits and lifestyle habits as measured by physical activity and nutrition instruments.
