帕金森病(PD)是常见的神经退行性疾病,起病隐匿,由于缺乏病理特异性的生物标记物,PD早期诊断困难,致使医学干预往往偏疾病后期,效果不佳。黑质多巴胺能神经元的退变是临床期PD的核心病理改变,受到了脑内、外多种病理因素的共同影响。既往研究多从单一维度来阐述黑质的退变,未综合考虑其病理生理改变的复杂性,在早期诊断中的价值有限。磁共振成像技术能够从不同角度观察与黑质退变相关的脑内改变,包括铁负荷测量和自由水含量评估;而外周血炎症因子可以反映脑外环境状态。因此,有机整合上述手段能够帮助全面研究与PD黑质退变相关的脑内、外因素,提升对早期PD病理生理改变的理解、促进临床转化。本项目延续了申请人前期的磁共振研究,拟通过构建早期PD患者的临床资料-磁共振影像-外周血炎症因子数据库,来研究能够表征早期PD的多维度指标;最后,整合有意义的信息来构建罹患早期PD的风险评估模型,以期为临床早期诊断提供客观依据。
英文摘要
Parkinson's disease (PD) is a common neurodegenerative disease with insidious onset. Due to the lack of pathologically specific biomarkers, early diagnosis of PD is difficult, resulting in medical interventions that are often late in the disease and have poor results. Degeneration of dopaminergic neurons in substantia nigra is the core pathology of clinical PD, which is affected by various pathological factors inside and outside the brain. Previous studies have mostly described the degeneration of substantia nigra from one single perspective, without comprehensively considering the complexity of its pathophysiological changes, and have limited value in early diagnosis. Magnetic resonance imaging (MRI) technology can observe changes in the brain related to substantia nigra from different views, including the measurements of brain iron and free water content; and peripheral blood inflammatory factors can reflect the state of the external brain environment. Therefore, the integration of the above methods can help to comprehensively study the internal and external factors related to PD substantia nigra, improve understanding of the pathophysiology of early PD, and promote clinical translation. This project continues the applicant's previous MRI researches. It is planned to study the multi-dimensional features that can characterize early PD by constructing clinical evaluation-MRI-peripheral blood inflammatory factor database. At last, this study will construct risk model for early PD by integrating these informative features that could have the ability to provide objective evidence for early clinical diagnosis.
