项目摘要

歯科診療時のエアロゾルの発生と拡散状況および、通常の室内換気と口腔内バキューム、口腔外バキュームによるエアロゾルの拡散防止効果について、移動ベクトルや飛散範囲を評価することを目的に、微粒子可視化システムと感水紙試験による解析を行なった。歯科用ファントム模型を使用し、エアータービン、電動回転切削器具(等速、5倍速)を用いた上顎前歯に対する処置を模擬的に実施した。吸引装置(口腔内バキューム、口腔外バキューム)使用の有無によるエアロゾルおよび飛沫の飛散状況を、微粒子可視化システムと感水試験紙を用いて観察した。吸引装置を使用しない場合、主に12 時方向への飛散がみられ、エアータービン使用時に最も顕著であった。口腔内バキューム使用時には、全方向で飛散量は減少し、口腔内バキュームと口腔外バキュームを併用して使用することで飛散量はさらに減少した。感水紙試験を用いた解析でも12 時方向への飛散がみられ、口腔内バキューム、口腔外バキュームとも、単独の使用でも一定の飛散量を減少させる効果が得られた。口腔内バキュームと口腔外バキュームの併用が、歯科治療時に発生するエアロゾルや飛沫の飛散量を顕著に減少させることが視覚的、定量的に確認された。以上の研究結果を国内学会で発表した。研究成果の一部は、日本歯科医学会監修の『医療関連感染対策実践マニュアル』改訂版に掲載予定である。
