项目摘要

本研究は、特別養護老人ホーム（特養）でのAdvance Care Planning（ACP）における看護職と医師による医学的妥当性と適切性判断にかかわる日常活動を、参与観察とインタビューによって明らかにしようとするものである。調査方法は参与観察とインタビューの2段階で構成される前向き観察研究である。調査に先立ち、本年度は主に文献レビューを実施した。本研究では、厚生労働省の「人生の最終段階における医療・ケアの決定プロセスに関するガイドライン」に基づいたACPの実践に着目している。このガイドラインは、患者の自己決定権が尊重されるために必要なプロセスを示すものとして概ね肯定的に捉えられてきた。一方、ガイドラインの射程、その運用と医学的無益性の概念との関係、診療報酬および介護報酬システムとACPの関係に注目すると、特養でのACPの問題点や課題が見えてくる。そのため、これらの視点を焦点化しながら文献レビューを行った。結果抽出された点は以下である。ガイドラインの射程を把握する際に重要なのは、その策定の目的が消極的安楽死（つまり「尊厳死」）のルールを公的に示すことにあったという点である。日本でいう「尊厳死」とは、事実上、治療を不開始または中止することを意味している。これをふまえるなら、このガイドラインの運用においては、治療の不開始・中止がどのように判断されるのかを注視することが重要である。「人生の最終段階」における無益性解釈は、「理」と「情」が混同される可能性がある。さらに、システムとACPの関係に着目するなら、現行の診療報酬・介護報酬には、ACPの導入によって医療の不開始や中止を「正解」として導く機能があると考えられる。これらの文献レビューの結果は、複数の研究会で発表し、発表資料はreseachmapで公表した。今後の参与観察とインタビューに用いるガイド作成に視点が与えられた。
