研究課題であるICU・ERにおける緩和ケアのアンメットニーズを調査するに先立ち、まずは現状どの程度ICUやERで医療を受ける患者やその家族が医療に満足しているのかを明らかにする必要があると考えた。そのため、2021年度はICUにおける患者満足度を測定する尺度の日本語版作成に取り組んだ。作成者より翻訳許可を得たうえで、トランスレーション・バックトランスレーションを行った。その後、クリティカルケアに精通しているエキスパートを多職種から抽出し、エキスパートパネルを実施した。エキスパートパネルにより、項目修正を行い、再度バックトランスレーションを行い、作成者に確認を行った。次の段階として、医療者・非医療者を対象に意味の了解さ、回答の容易さを調査し、1回修正を行った。その後、バイリンガルテストを海外で勤務している医師・看護師・理学療法・助産師など計５名に対して実施し、日本語版と英語版の回答の一致率を確認した。重みづけΚ係数は0.52と中程度であることが確認できた。現在は、次の段階としてICUの患者を中心にパイロット調査を行う準備を進めている段階である。なお、ICUやERの患者特性を配慮し、家族代理評価可能性も同時に検証する予定である。くわえて、クリティカルケア領域は患者に意識がなく、家族が代理意思決定などを行うことも少なくない。そのため、患者とは独立して家族の視点から患者満足度を評価してもらい、患者との一致率を確認することで家族の代理評価可能性も一緒に検証することを予定している。このパイロット調査が終了次第、全国のICUやERにおいて調査を行うとともに、評価が低かった患者やその家族から聞き取り調査を行い、満足できなかった要因を明らかにしていく計画である。
